Dear Parents and Carers

You may be aware that new legislation regarding data, known as GDPR (General Data Protection Regulation) comes into force on 25th May.
As part of our GDPR compliance process, Epping Upland C.of E. Primary School has reviewed all the personal data that we collect as a data controller. We have identified the relevant legal bases and in the below circumstances, we require your consent to process your personal data.
We are required by GDPR to use clear, plain language that is easy to understand. Therefore, if you require anything within this form to be explained, please make contact with us before completing it.
Periodically, we would like to contact you for the following reasons as attached. You are not obliged to agree to any of them and if you refuse to consent, it will not be to your detriment.

Your completed consent form will be kept as a record of your decisions and from time to time we will contact you again on this.
In relation to your personal data that you provide, we will only use it for the below reasons that you agree to. It will not be shared with any third parties.
We will regularly review consent to check that the relationship, the processing and the purposes have not changed. Specifically, after the pupil has completed Key Stage 1, we will contact you again to refresh the below consent.
As a result of these new regulations, there will be certain aspects within our procedures that may have to be changed or modified and we respectfully request that you show understanding in this. If you have any questions, please do not hesitate to contact me.
Please return the Consent Form to the school office in a sealed and clearly marked envelope by Friday 11th May 2018.
